
Appointment types and services

INITIAL ASSESSMENT

Initial assessments are booked as 90 minutes

You will spend 60 minutes with your clinician evaluating your history and putting the pieces together

The remaining 30 minutes will be spent creating an individualised summary of your session and providing information relevant to you and your treatment plan (this may include information to read, recommendations of health professionals to contact, tools, items to consider purchasing)

Please ensure you bring a notepad and pen as what I teach and describe to you; we want you to be able to write this in your own words as we all learn and interpret information differently
SUBSEQUENT SESSION

Subsequent sessions are 50 minutes

30-minute sessions are available for those who find longer appointments challenging

NDIS
-
We do see patients on the NDIS, however we are not a 'registered provider'
-
Please ensure you already have approval for Exercise Physiology, otherwise you will be responsible for the fees

We cannot take on appointments for participants who are ONLY seeking reports. This is not the type of practice we run and nor does we have the time in the practice for extensive reports

Please note that NDIS reports need to be scheduled into the calendar and will be billed for all time spent on researching, evaluating and interpreting questionnaires, liaising with other health practitioners and completing the report +/- making edits as requested
LETTERS & REPORTS - ie: school/ work / GP / specialist

We can complete letters and reports however these will need to be scheduled into the calendar and will incur a fee
CLINICAL MENTORING
Jacqui has a huge passion for teaching others working with invisible illness clients. Whether this is just starting out working with this population or whether you want to discuss particular patients you’re working with to ensure you’re giving them the best possible therapy or assistance, Jacqui will be able to work with you via Zoom.
MEDICARE
-
You will most likely be eligible for a GP chronic condition management plan (GPCCMP). This term is new since mid 2025. Unless otherwise specified by the referring GP, referrals to allied health services for patients with a chronic condition will be valid for 18 months. The number of sessions is no longer required to be written by the GP which gives the patient more flexibility to use their plan. HOWEVER, please note you only get 5 sessions per plan and we must know how many you want to use for your services with Dizzy hEDS & Co. If you use a Medicare service with another practitioner without us being made aware, you will not receive a rebate for that session.

The rebate is $61.80 back from Medicare. This will be more if over the Medicare threshold.

You may need to advise your GP clinic when you make an appointment for the GPCCMP, as many clinics have different procedures on how they develop these, and you will need a longer appointment and/or an appointment with the GP and Nurse.

PRIVATE HEALTH INSURANCE
You may be covered for Exercise Physiology services with your private health insurance. Please check with your fund as the level of cover and conditions can vary from one insurer to another.
*Please be aware that you can only claim on Medicare OR private health insurance for each session. It is not possible to make a claim with both for one session.

** Please note that lengthy reports and correspondence may incur a fee to be read and summarised by your clinician
